Forms

• Major Electives and online petition form to submit outside-department courses for consideration and approval.
  
• Informatics Internship (INFO 495)
  • INFO 495 guidelines and request form. Submit completed form, with necessary signatures, to the iSchool Student Services Office (MGH 470). Add code for INFO 495 will be distributed upon approval of the arrangement.
• Independent Study in Informatics (INFO 499)
  • INFO 499 request form. Submit completed form, with necessary signatures, to the iSchool Student Services Office (MGH 470). Add code for INFO 499 will be distributed upon approval of the arrangement.
